Consent to the processing of personal data

In accordance with the Act no. 101/2000 Coll., On protection of personal data and on amendments of some Acts as amended (hereinafter referred to as the “Act”) I hereby agree to the processing of my contact information which I've now provided to the Institute of Documentary Film, z.s. (Incorporated Club) in Prague, Domicile Prague Štěpánská 14 Postal Code 110 00, ID (IČO) 26 54 98 67, entered in the Commercial Register kept by the Municipal Court in Prague, Section L, File 12359, as personal data administrator or by entities contractually authorized by the administrator to process personal data for the purpose of communicating information, conduct applicant satisfaction surveys as well as to disseminate information by electronic means pursuant to Act no. 480/2004 Coll., on certain Information Society Services. Personal data may be processed manually and automatically by employees of the personal data administrator or a processor of personal data. I give my consent to the processing of my personal data voluntarily and have been instructed in accordance with the law of all my rights under the law, especially the right of access to personal data, the right to correct personal data, the rights in accordance with Section 21 of the Act, as well as the right to withdraw such consent at any time by a written information sent to the address of the Administrator. I agree to receive commercial information sent by the administrator to my email address, which I voluntarily provided. In the case of disclosing the personal data of a third party I declare that the third party agrees with disclosing its personal data. Should the right to privacy be violated a complaint may be filed with the Office for Personal Data Protection. The consent to the processing of personal data by the user is given for an indefinite period.
